Clinical service partnerships

The Centre for Innovation and Education in Aged Care (CIEAC) is working closely with key clinical service partners to disseminate the Tri-focal Model of Care in residential aged care settings, as a major aged care workforce development initiative in Australia.

The clinical service partnerships will further develop the Teaching Nursing Home initiative, building capacity in research and knowledge translation in residential aged care facilities. In particular, potential Honours, Masters and PhD students from Deakin University will be invited to undertake research related to the Tri-focal Model of Care with support from the Centre for Nursing Research – Deakin University and Monash Health Partnership and the Cabrini-Deakin Centre for Nursing Research.

Additionally, Deakin University nursing students will be offered clinical placements within each of the participating residential aged care facilities. They will be exposed to an improved learning environment characterised by clinical practices that are evidence-based and consistent with University curricula, and a positive, person-centred approach to resident care. Students will be mentored/preceptored by nurses within each facility who have completed the Tri-focal Model of Care facilitator training.

Cabrini, Monash Health and Western District Health Service are consortium members of the Tri-focal Model of Care: Teaching and Research Aged Care Services project.

Alfred Health is a leader in health care delivery, improvement, research and education. We strive to achieve the best possible health outcomes for our patients and our community by integrating clinical practice with research and education. Caulfield Hospital provides a range of specialty services in the areas of community services, rehabilitation, aged care, residential care and aged mental health. The CIEAC is working with the Montgomery and Namarra aged care facilities.
Cabrini is a multi-site healthcare service with more than 800 beds offering a wide range of acute services including palliative care, rehabilitation and residential aged care. The CIEAC is working with the residential aged care facility at Ashwood.
Monash Health is a major teaching health service provider of acute, subacute and residential aged care services. The aged care service comprises approximately 300 beds, providing low, high, respite and dementia care, as well as specialist aged mental health care services. The CIEAC is working with Allambee and Yarraman aged care facilities. These residential aged care facilities participate in undergraduate, graduate and continuing education training programs to develop their workforce capacity.
Western District Health Service (WDHS) is located in Penshurst, Hamilton and Coleraine in western Victoria. The service has four residential aged care facilities that comprise a total of 175 beds. The CIEAC will be working with The Grange and The Birches facilities in Hamilton. WDHS conducts graduate programs for both Registered Nurses and Enrolled Nurses, and has recently extended the programs to two years, allowing greater employment opportunities for graduates. WDHS offers graduates the opportunity to complete their placements across all sites, giving them valuable insights into working within small, remote communities, as well as speciality areas like dementia care.
